29 CFR 1910.28(b)(3)(ii): The employer did not ensure each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: a) International Waxes, Inc. at 45 Route 446 in Smethport, PA - Wax Tub Area. On or about March 19, 2018, and times prior thereto, the metal grate covering the drain in the Wax Tub Area was removed. Employees were exposed a to 3-foot fall into scalding hot water draining from the wax tub.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a) International Waxes, Inc. at 45 Route 446 in Smethport, PA. On or about March 23, 2018, and at times prior there to, an in-patient hospitalization was not reported to OSHA within 24 hours. The accident and resulting in-patient hospitalization took place on or about March 19, 2018, at approximately 9:35AM. The incident was not reported to OSHA until March 20, 2018, at approximately 4:20PM.
