29 CFR 1910.212(a)(3)(ii): Point of operation of machine was not guarded to prevent employee from having any part of their body in the danger zone during operating cycle: a) At the time of the inspection, the Lever material cutter guard did not prevent the operator from operating the machine with the guard raised. The control panel key was left in the machine which allows the operator to run the machine while exposed to the cutting blade.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: a) At the time of the inspection, multiple pendant drops were documented that were not equipped with strain relief. One specific drop above the Milwaukie chain hoist had exposed wires where the outer insulation had pulled away from the junction box.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) At the time of the inspection, the employer did not have machine specific procedures readily available for the Lever Material Cutter. The cutting blade for the cutter was replaced approximately every two years. This cutter was the only cutter that did not have a cord and plug electrical disconnect.
