29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: a) 33 Eberly Place, Fords, NJ: The employer did not ensure that safe and acceptable means are used to locate underground utilities while performing trenching work in close proximity to an active gas line. On or about 3/19/18 an excavator struck a gas line while performing trenching work to install a new sewer line.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a) 33 Eberly Place, Fords, NJ: Employees who were performing sewer line work in an excavation approximately 11 feet deep were not protected by an adequate cave-in protection system. This hazard occurred on 3/19/18.
