29 CFR 1926.201(a): Signaling by flaggers and the use of flaggers, including warning garments worn by flaggers did not conform to Part VI of the Manual on Uniform Traffic Control Devices, (1988 Edition, Revision 3 or the Millennium Edition),       a) 400 Saint Louis Street: North Hamilton Street & Saint Anthony Street (Intersections/Roadway Area):  On or about March 21, 2018, and at times prior; the "controlling employer" Rogers & Willard, Inc. failed to ensure his employee(s) and the employee(s) of the sub-contractor Burroughs Masonry were provided high-visibility apparel for working as flaggers in a traffic work zone thus exposing them to struck-by hazards from vehicular traffic.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    a) At 400 Saint Louis Street (Northwest Corner: Back Wall Area/North Hamilton Street): On or about March 21, 2018, and at times prior; the "controlling employer" Rogers & Willard, Inc. failed to ensure employee(s) of the sub-contractor Burroughs Masonry were using a body belt with lanyard attached to the approved lanyard anchor point while performing tuckpointing operations exposing employee(s) to catapult hazards and fall hazards of approximately fifteen (15) feet to the concrete or ground below.
