29 CFR 1926.251(c)(4)(iv): Wire rope used in any length of eight diameters, the total number of visible broken wires exceeded 10 percent of the total number of wires, or the rope showed signs of excessive wear, corrosion, or defect. (a) Yellowhawk Tribal Health Center - On or about March 22nd, 2018 and at times prior thereto, the employer did not ensure that the 3' wire rope sling (3/8" diameter), used for the lifting of the Miller Trailblazer (95479) welding unit, did not show signs of excessive wear and damage.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. (a) Yellowhawk Tribal Health Center - On or about March 22nd, 2018 and at times prior thereto, the strain relief was damaged on the relocateable power tap (RTP) for the Milwaukie Battery charging station. (b) Yellowhawk Tribal Health Center - On or about March 22nd, 2018 and at times prior thereto, the strain relief on the green extension cord, energizing the DeWalt grinder, was damaged.
