29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:           a) Exterior of Building - Back Roof: Employee was exposed to head injury while engaged in roofing activity as another employee was removing shingles above him, on or about 3/24/18.   b) Exterior of Building - Back Deck: Employee was exposed to head injury while engaged in cleaning activity as another employee was installing shingles above him, on or about 3/24/18.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Exterior of Building - Roof Back: Employees without proper means of fall protection were exposed to fall hazards of approximately 25 feet while engaged in roofing activities , on or about 3/24/18.  b) Exterior of Building - Lower Roof Back: Employees without proper means of fall protection were exposed to fall hazards of approximately 15 feet while engaged in roofing activities , on or about 3/24/18.  c) Exterior of Building - Roof Front: Employee without proper means of fall protection was exposed to fall hazards of approximately 15 feet while engaged in roofing activities , on or about 3/24/18.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) Exterior of Building - Roof Back: Employees were exposed to approximately 15 foot fall while utilizing a ladder that extended less than 2 feet above the surface to access the roof, on or about 3/24/18.
