29 CFR 1910.178(l)(3)(ii)(D): Pedestrian traffic in areas where the vehicle will be operated; On or about 3/26/2018 an employee was operating a wheel mounted material handler at a scrap yard and was in the process of loading a vehicle onto a flatbed trailer. During this process the operator smashed the car several times with a grapple attachment, the last impact caused an explosion resulting in a fire spreading throughout the pile of scrap metal nearby. The operator tried to extinguish the vehicle by removing it from the pile, this resulted in the spreading of the fire. Other employees and three pedestrians began using fire extinguishers to try to put the fire out while in the swing radius of the material handler Liebherr A 934 C ERC Scrap Material Handler.

29 CFR 1910.178(l)(3)(ii)(I): Other unique or potentially hazardous environmental conditions in the workplace that could affect safe operation. On 3/26/2018 employees were exposed to hazards associated with fighting a fire while a material handler, the Liebherr A 934 C ERC Scrap Material Handler and the Volvo L90E Wheel Loader was being operated in the same area filled with smoke reducing visibility exposing employees to a struck by hazard.
