29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. Family Dollar, Project 17.146, Rt. 264, Window Rock, AZ: Three employees were not wearing hard hats at the construction site when overhead hazards were present.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: Family Dollar, Project 17.146, Rt. 264, Window Rock, AZ: Numerous number 4 rebar protruding up to 24" above ground were not protected. Employees that placed form work and walk in this area as well as employees working from a 15' roof top without fall protection are exposed to impalement hazards.
