29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    a.) Imperial Carbide, Inc., Production areas, on or about March 28th, 2018 and times prior thereto:  Employees worked with and around hazardous substances including, but not limited to, DH-38 degreaser cleaner, kerosene, and isopropyl alcohol during work operations at the facility.  The employer did not provide effective hazard communication training for the employees working with and around hazardous substances.

29 CFR 1910.106(d)(2)(i): Approved containers and/or portable tanks were not used for the storage of flammable liquids:    a.)    Imperial Carbide, Inc., Finishing area of the facility, on or about March 28th, 2018 and times prior thereto:  The employer did not store a category 3 flammable liquid (kerosene) in an approved container.  The kerosene was stored in a plastic spray bottle onsite.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:    a.) Imperial Carbide, Inc., Finishing area of the facility, on or about March 28th, 2018 and times prior thereto:  Secondary containers of products including, but not limited to, kerosene were not labeled with the hazards of the hazardous chemical(s) contained therein.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a.)  Imperial Carbide, Inc., West wall of production area, on or about March 28th, 2018 and times prior thereto:  Employees worked with and around corrosive chemicals including, but not limited to, DH-38 Degreaser Cleaner.  The emergency eyewash/shower station (Non-Pressurized Self-Contained Eyewash) in the eyewash/shower station was not in compliance with ANSI/ISEA 358.1-2014 for Emergency Eyewash and Shower Equipment in that:     1.  Weekly visual inspections of the eyewash station were not conducted. The emergency eyewash unit was not visually inspected weekly to verify the flushing fluid does not need to be changed or supplemented.
