On the Card Line Platform, electrical extension cords were placed across the tread of the stair exposing employees to tripping hazards.

29 CFR 1910.25(b)(3): General Requirements. The employer must ensure stairs have uniform riser heights and tread depths between landings; (a) On the Card Line Platform, the stairs leading to the platform did not have uniform riser heights exposing employees to tripping hazards. (b) On the Thermal Former T1 north platform, the west side stairs leading to the platform did not have uniform riser heights exposing employees to tripping hazards. (c) On the Thermal Former T1 south platform, the west side stairs leading to the platform did not have uniform riser heights exposing employees to tripping hazards. (d) On the Thermal Former T1 south platform, the east side stairs leading to the platform did not have uniform riser heights exposing employees to tripping hazards. (e) On the Thermal Former T2 south platform, the west side stairs leading to the platform did not have uniform riser heights exposing employees to tripping hazards. (f) On the Thermal Former T2 south platform, the east side stairs leading to the platform did not have uniform riser heights exposing employees to tripping hazards.

The employer must ensure each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as follows: (see TABLE D-2) The Card Line Platform had four risers used to access the platform which did not have the stair rail system and handrail exposing employees to trip hazards.
