29 CFR  1910.1000(a)(2):  An employee's exposure to any substance in Table Z-1, the exposure limit of which is not preceded by a "C", shall not exceed the 8-hour Time Weighted Average (TWA) given for that substance any 8-hour work shift of a 40-hour work week:  At a facility located at 851 Sullivan Drive, Fond du Lac, WI 54935:  a)  At the Production Line, a Production Line Lead was exposed to carbon monoxide at a level of 273 ppm as an 8-hour TWA on July 27, 2020, approximately 5.5 times the PEL of 50 ppm. This exposure was determined by a carboxyhemoglobin (COHb) specimen collected after a 420 minute exposure with zero exposure being assumed for the remaining 60 minutes of an 8-hour work shift.  b)  At the Warehouse, a Custodian was exposed to carbon monoxide at a level of 78.8 ppm as an 8-hour TWA on July 27, 2020, approximately 1.6 times the PEL of 50 ppm. This exposure was determined by a carboxyhemoglobin (COHb) specimen collected after a 240 minute exposure with zero exposure being assumed for the remaining 240 minutes of an 8-hour work shift.  c)  At the Production Line, a Line Assembly Person was exposed to carbon monoxide at a level of 123 ppm as an 8-hour TWA on July 27, 2020, approximately 2.5 times the PEL of 50 ppm. This exposure was determined by a carboxyhemoglobin (COHb) specimen collected after a 450 minute exposure with zero exposure being assumed for the remaining 30 minutes of an 8-hour work shift.  d)  At the Die Cut/Print Line, a Die Cut Assistant Operator was exposed to carbon monoxide at a level of 219 ppm as an 8-hour TWA on July 27, 2020, approximately 4.4 times the PEL of 50 ppm. This exposure was determined by a carboxyhemoglobin (COHb) specimen collected after a 420 minute exposure with zero exposure being assumed for the remaining 60 minutes of an 8-hour work shift.  e)  At the Production Line, a Production Associate was exposed to carbon monoxide at a level of 140 ppm as an 8-hour TWA on July 27, 2020, approximately 2.8 times the PEL of 50 ppm. This exposure was determined by a carboxyhemoglobin (COHb) specimen collected after a 420 minute exposure with zero exposure being assumed for the remaining 60 minutes of an 8-hour work shift.  f)  At the Production Line, a Production Associate was exposed to carbon monoxide at a level of 176 ppm as an 8-hour TWA on July 27, 2020, approximately 3.5 times the PEL of 50 ppm. This exposure was determined by a carboxyhemoglobin (COHb) specimen collected after a 450 minute exposure with zero exposure being assumed for the remaining 30 minutes of an 8-hour work shift.  g)  At the Die Cut/Print Line, a Die Cut Operator was exposed to carbon monoxide at a level of 134 ppm as an 8-hour TWA on July 27, 2020, approximately 2.7 times the PEL of 50 ppm. 29 CFR  1910.1000(e):  To achieve compliance with paragraphs (a) through (d) of this section, administrative or engineering controls must first be determined and implemented whenever feasible. When such controls are not feasible to achieve full compliance, protective equipment or any other protective measures shall be used to keep the exposure of employees to air contaminants within the limits prescribed in this section. Any equipment and/or technical measures used for this purpose must be approved for each particular use by a competent industrial hygienist or other technically qualified person. Whenever respirators are used, their use shall comply with 1910.134:   At a facility located at 851 Sullivan Drive, Fond du Lac, WI 54935; the employer did not implement engineering or administrative control to reduce employees' exposure to carbon monoxide generated by the Yale LP powered forklift (M# B401C-180D, S#B40/48IC-412B-03359) when:  a)  At the Production Line, a Production Line Lead was exposed to carbon monoxide at a level of 273 ppm as an 8-hour TWA on July 27, 2020, approximately 5.5 times the PEL of 50 ppm. Remove from service any forklifts that exceed acceptable levels of carbon monoxide in their exhaust.  2)  Install, inspect and maintain industrial purpose carbon monoxide meters in portions of the facility where combustion equipment (LP vehicles, gas powered tools, heating units, furnaces, boilers, etc.) have the potential for releasing combustion by-products into the work area.  3)  Increase or install general dilution ventilation for mobile sources of carbon monoxide (e.g. LP powered vehicles). Install local exhaust ventilation or fresh air supply at fixed locations (e.g. exhaust capture hoods at boilers or fresh air islands to work stations).
