29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  Plant no. 9 (Streator, Illinois), selecting area, H2 FP3 inspection line, infeed conveyor - on or about March 22, 2018, process checkers/inspectors that were assigned to perform servicing and maintenance activities on energized equipment were provided with hazardous energy control (lockout tagout) procedures that were not adequate. The procedure specifically excluded tasks that necessitated hazardous energy control, including glass clearing and equipment cleaning tasks. Employees engaged in glass clearing and equipment cleaning tasks were exposed to caught-in hazards likely to result in amputation due to in-going nip points because the techniques listed were inaccurate and disorderly.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  Plant no. 9 (Streator, Illinois), selecting area, H2 FP3 inspection line, infeed conveyor - on or about March 22, 2018, process checkers/inspectors assigned to perform servicing and maintenance activities that required the use of lockout/tagout were not trained on safe application, usage, methods, and means to isolate and control hazardous energy sources as they were permitted to rely on emergency stop control. Employees were assigned to clear glass from the infeed conveyor's components that exposed them to in-going nip points.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    (1) prepare for shut down by recognizing energy hazards and its controls,  (2) turn off and shut down the machines,  (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (4) affix lockout or tagout devices to each energy isolating device,  (5) render safe all potentially hazardous stored or residual energy,  (6) verify that isolation and de-energization of the machine or equipment have been accomplished.    Plant no. 9 (Streator, Illinois), selecting area, H2 FP3 inspection line, infeed conveyor - on or about March 22, 2018, process checkers/inspectors that were assigned to clear glass from the infeed conveyor's components were exposed to in-going nip points while hazardous energy controls (lockout) were not done in the sequence as required by 29 CFR 1910.147(d)(1)-(6).    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.303(g)(2)(ii): In locations where electric equipment is likely to be exposed to physical damage, enclosures or guards shall be so arranged and of such strength as to prevent such damage.    (a) Plant no. 9 (Streator, Illinois), selecting area, H2 FP3 inspection line, operator's single stop control box: on or about March 27, 2018, process checkers/inspectors were exposed to contact with live wires likely to result in serious injury when walking-by and performing general housekeeping tasks around taped, terminated ends of a flexible cable. The termination point was not guarded against accidental damage and contact by use of approved cabinets or enclosures.    (b) Plant no. 9 (Streator, Illinois), selecting area, H2 FP3 inspection line's infeed conveyor: on or about March 27, 2018, process checkers/inspectors were exposed to contact with exposed live wires likely to result in serious injury when walking-by and performing general housekeeping tasks around a plugged-in flexible cable that had its outer jacket insulation damaged by the plug connection.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
