29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. At the location of 7939 Floyd St. in Overland Park KS, employees engaged in the installation of windows with the use of an articulating boom were exposed to fall hazards of approximately 24 feet to the ground below. The employees were exposed in that the lanyards on the employees were not anchored to the anchor point to keep them from falling out.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: At the location of 7939 Floyd St. in Overland Park KS, an employee installing siding around the windows was exposed to fall hazards of approximately 8 feet to the ground below. The employee was exposed in that a fall protection system was not in place to keep him from falling to the lower level.
