29 CFR 1904.31(b)(2): The employer did not record injuries and illnesses for employees obtained from a temporary help service, employee leasing service, or personnel supply service, and were supervised by the employer on a day-to-day basis: Located at the Christmas Tree Shops Inc., 270 Daniels Way, Burlington, New Jersey: On or about 6/25/2018, the employer did not record the following workplace injuries and illnesses on the OSHA 300 Log for the calendar year 2018; a) On or about 3/1/2018, an employee placed by First Step Staffing, received a back injury after a fall from a ladder onto the rolling line, resulting in restricted work activity. b) On or about 3/26/2018, an employees placed by ABACUS Staffing, amputated the tips of three fingers after they came into contact with an unguarded chain and sprocket. c) On or about 3/30/2018, an employee placed by First Step Staffing, sprained the right knee after working with a broken conveyor system, resulting in restricted work activity. d) On or about 3/30/2018, an employee placed by First Step Staffing, sprained the left wrist after boxes fell on it while stacking them on a conveyor, resulting in lost time. e) On or about 5/2/2018, an employee placed by First Step Staffing, experienced lower back pain after working in the order selection department of the warehouse, resulting in lost time. f) On or about 5/15/2018, an employee placed by First Step Staffing, slipped on oil and fell resulting in restricted work activity. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
