29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used: a) On or about 3/29/18 at a roof replacement project located at 94 Oliver Street, Lockport, New York; employees were using a ladder jack scaffold, with the scaffold brackets and platform to the outside of the supporting ladders. The employees accessed the platform by climbing the supporting ladders and climbing around the bracket and platform. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 3/29/18 at a roof replacement project located at 94 Oliver Street, Lockport, New York; employees working on the roof were removing shingles from a two story house. The roof had a 5 in 12 pitch and an eave heights of approximately 20 feet. There was no fall protection in use. NO ABATEMENT CERTIFICATION REQUIRED Mike Ignatowski & Sabrina Hancock dba A Top Level Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 1275051, citation number 1, item number 1a and was affirmed as a final order on 1/5/18, with respect to a workplace located at 128 Walnut Street, Lockport, New York.
