Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. a. On or about March 30, 2018, the employer did not ensure that the clamp attachments installed on the Toyota propane powered forklifts were approved by the forklift manufacturer prior to use in the workplace.

29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). a. On or about March 30, 2018, the employer did not ensure that each employee operating Toyota propane powered forklifts had completed the forklift training program, to include formal instruction and evaluation, prior to permitting them to operate in the warehouse, thereby exposing employees to struck by and crushed by hazards.
