29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: (a) Roof of Structure, Left Rear Corner - Worker guiding into place prefab A frame roof trusses that were being hoisted by a boom truck crane was not wearing a hard hat to protect him from the swinging material lifted by the crane, on or about 3/29/2018. O.H. Construction LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1033612, citation number 1, item number 1 and was affirmed as a final order on 4/10/2015, with respect to a workplace located at 4937 Western Ave NW Washington DC 20016.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Roof of Structure, Left Rear Corner - Worker guiding into place prefab A frame roof trusses that were being hoisted by a boom truck crane was not wearing fall protection and exposed to a fall of approximately 30 feet, on or about 3/29/2018. The OH Construction LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1033612, citation number 1, item number 2 and was affirmed as a final order on 4/10/2015, with respect to a workplace located at 4937 Western Avenue NW Washington DC 20016.
