29 CFR 1910.95(d)(1): When information indicates that any employee's exposure may equal or exceed an 8-hour time-weighted average of 85 decibels, the employer shall develop and implement a monitoring program.    a. The employer did not develop and implement a noise monitoring program for employees working in the pallet builder area. An employee working in the pallet builder area was exposed to continuous noise at 125.6% on April 10, 2017, which exceeded the action limit (AL) of 50%, an 8-hour TWA (time weighted average) sound level of 85 dBA. The employee's 8-hour TWA sound level was approximately 91.6 dBA during the 433 minute sampling period. Time weighted average includes a zero increment for the 47 minutes not sampled.    b. The employer did not develop and implement a noise monitoring program for employees working in the pallet builder area. An employee working in the pallet builder area was exposed to continuous noise at 129.2% on April 10, 2017, which exceeded the action limit (AL) of 50%, an 8-hour TWA (time weighted average) sound level of 85 dBA. The employee's 8-hour TWA sound level was approximately 91.8 dBA during the 425 minute sampling period. Time weighted average includes a zero increment for the 55 minutes not sampled.    c. The employer did not develop and implement a noise monitoring program for employees working in the pallet builder area. An employee working in the pallet builder area was exposed to continuous noise at 129.8% on April 10, 2017, which exceeded the action limit (AL) of 50%, an 8-hour TWA (time weighted average) sound level of 85 dBA. The employee's 8-hour TWA sound level was approximately 91.9 dBA during the 429 minute sampling period. Time weighted average includes a zero increment for the 51 minutes not sampled.    d. The employer did not develop and implement a noise monitoring program for employees working in the band saw area. An employee working in the band saw area was exposed to continuous noise at 167.0% on April 10, 2017, which exceeded the permissible exposure limit (PEL) of 100%, an 8-hour TWA (time weighted average) sound level of 90 dBA. The employee's 8-hour TWA sound level was approximately 93.7 dBA during the 451 minute sampling period. Time weighted average includes a zero increment for the 29 minutes not sampled.    e. The employer did not develop and implement a noise monitoring program for employees working in the chop saw area. An employee working in the chop saw area was exposed to continuous noise at 260.2% on April 10, 2017, which exceeded the permissible exposure limit (PEL) of 100%, an 8-hour TWA (time weighted average) sound level of 90 dBA. The employee's 8-hour TWA sound level was approximately 96.9 dBA during the 466 minute sampling period. Time weighted average includes a zero increment for the 14 minutes not sampled.

The employer did not develop and implement an audiometric testing program for employees working in the pallet builder area.
