On or about March 30, 2022, and at times prior, the energy control procedure utilized for service and maintenance tasks on the Denailer Bandsaw did not specifically outline the scope, purpose, authorization, rules, and techniques to be utilized to control the hazardous energy associated with the machine. The procedure in place did not have a scope, purpose, authorization, or the specific steps and techniques necessary to control the electrical, gravitational, and pneumatic energy sources, thereby exposing employees to caught-in, and struck-by hazards.

On or about March 30, 2022, and at times prior, authorized employees performing saw blade changes on the Denailer Bandsaw were not effectively training on the recognition of hazardous energy sources, the type and magnitude of the energy sources, and the methods and means necessary for energy isolation and control. 29 CFR  1910.212(a)(5): Exposure of blades. When the periphery of the blades of a fan is less than seven (7) feet above the floor or working level, the blades shall be guarded. The guard shall have openings no larger than one-half (1/2) inch.  a. On or about March 30, 2022, and at times prior, the employer did not ensure that the portable fan by the Denailer Bandsaw had its metal blades guarded in that the front guard of the blade was not in place. On or about March 30, 2022, and at times prior, the employer did not ensure that the grounding path was permanent, continuous, and effective, for a portable metal fan in use by the Denailer Bandsaw in that the ground pin was not in place on the cord and plug connected equipment. As a result, employees were exposed to struck-by hazards.  b. On or about March 30, 2022, and at times prior, the employer did not ensure that the grounding path was permanent, continuous, and effective, for a portable radio in use by the Denailer Bandsaw in that the ground pin was not in place on the cord and plug connected equipment. 29 CFR  1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  a. On or about March 30, 2022, and at times prior, the employer did not ensure that the energy control procedure for the Denailer Bandsaw was periodically inspected at least annually to ensure the procedure was sufficient and being followed. As a result, employees were exposed to caught-in and struck-by hazards.  GC Pallet and Storage Inc was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA inspection number 1291627, citation number 1, item number 1, and was affirmed as a final order on May 3, 2018, with respect to a workplace located at 185 N Yale Avenue, Columbus, Ohio 43222.
