The employer did not provide forklift training and evaluation to the forklift operators prior to operating the forklift.

The electrical box was not used in accordance to its listing and labeling by being installed on the building structure.  Abatement Note: One feasible means of abatement is to replace the electrical box having knockouts with a pendant box without any knockouts.
