29 CFR 1910.269(l)(8)(v): The employer did not ensure that each employee exposed to hazards from electric arcs wore protective clothing and other protective equipment that had an arc rating greater than or equal to the heat energy estimated under paragraph (l)(8)(ii) of this section whenever that estimate exceeded 2.0 cal/cm2. This protective equipment shall cover the employee's entire body, except as listed in paragraphs (A) - (E): (a) On or about 3/16/2018 - Cooling Tower Lift Pump Station 6.9 KV Cooling Tower Board B Compartment 2, the employer did not ensure that an employee was wearing protective clothing and other protective equipment that had a minimum arc rating of 12.4 cal/cm2 when he was within the 16.5 foot arc flash boundary, the compartment door was open, and 6.9 KV cable continuity checks were being performed at the ground and test device GT-22 (ground truck). (b) On or about 3/16/2018 - Cooling Tower Lift Pump Station 6.9 KV Cooling Tower Board B Compartment 2, the employer did not ensure that an employee was wearing protective clothing and other protective equipment that had a minimum arc rating of 12.4 cal/cm2 when he was within the 16.5 foot arc flash boundary, the compartment door was open, a foreign material inspection was being conducted, and the shutter panel was open or being manipulated.
