29 CFR 1910.333(a)(1): The employer failed to ensure that live parts were deenergized before employees began to work on or near live parts. (a) Unit 2 Reactor Building: On or about 2/01/2024, the employer exposed employees to arc flash and electrocution hazards, in that procedures were not utilized or enforced to ensure Temporary Power Unit was not deenergized prior to work and the cord energized at 480 V weas not under exclusive control of the crew. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not using electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed: (a) Unit 2 Reactor Building: On or about 2/01/2024, the employer exposed employees to arc flash and electrocution hazards, in that electrical protective equipment, such as but not limited to, 8 cal pants, 8 cal shirt with sleeves down, Arc Rated face shield or Flash Suit and hood were not used when disconnecting Temporary Power Unit cord energized at 480 V prior to relocating unit. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
