29 CFR 1926.1053(b)(16): Portable ladders with structural defects were not immediately marked "Defective, Do not Use" and withdrawn from service until repaired: a) Employees were using a damaged ladder; a 6 ft aluminum A-frame ladder had a broken crossbrace. The ladder was used by employees constructing an 8 ft tall shed near the roof edge.
