29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Jobsite: Employees were working on a roof over 6 feet from ground level without use of an adequate personal fall arrest system. There were two workers using a body belt in place of a body harness during roofing operations.

29 CFR 1926.1053(b)(6): Ladders shall be used only on stable and level surfaces unless secured to prevent accidental displacement:  Jobsite: A portable ladder was not secured when it fell over due to windy weather conditions, while an employee was standing on the ladder.
