29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about April 4, 2018, at 2813 Shoals Hill Court, Dacula, Georgia:  Five employees performing framing work were exposed to struck-by hazards from falling tools and materials of the employees working above them when they worked beneath the other employees without wearing head protection.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  On or about April 4, 2018, at 2813 Shoals Hill Court, Dacula, Georgia:  Six employees performing framing work were exposed to struck-by hazards from saw dust, wood chips, and nails when they operated circular saws and nail guns without wearing eye protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about April 4, 2018, at 2813 Shoals Hill Court, Dacula, Georgia:  Four employees performing framing work were exposed to a fall hazard of approximately 11.5-feet and greater when they used an aluminum ladder that was not extended three feet above the second floor landing.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about April 4, 2018, at Lot 56, 2813 Shoals Hill Court, Dacula, Georgia:  Four employees performing framing work were exposed to a fall hazard of approximately 22.6-feet and greater when they worked from the trusses and at the unprotected edges of a house without using fall protection.  Moises Degante dba ALK Framing Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1068191, citation number 1, item number 1, and was affirmed as a final order on September 23, 2015, with respect to a workplace located at 511 Starling View Circle, Grayson, Georgia.
