29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about April 4, 2018, at 2843 Shoals Hill Court, Dacula, Georgia:  Five employees performing framing work were exposed to struck-by hazards from falling tools and equipment from the employees working above them when they worked beneath those employees without wearing hard hats.

On or about April 4, 2018, at 2843 Shoals Hill Court, Dacula, Georgia:  An employee performing framing work was exposed to struck-by hazards from saw dust, wood chips, and nails while using a Makita 5007F circular saw without using eye protection.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:ALLEGED VIOLATION DESCRIPTION (A.V.D.):   On or about April 4, 2018, at 2843 Shoals Hill Court, Dacula, Georgia:  a) An employee performing framing work was exposed to electric shock hazards while using a flexible electric cord that had multiple cuts to the outer insulation.   b) An employee performing framing work was exposed to electric shock hazards while using a flexible electric cord that had been improperly repaired with the outer sheathing removed leading into a plug used to repair the cord.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about April 4, 2018, at 2843 Shoals Hill Court, Dacula, Georgia:  Three employees performing framing work were exposed to a fall hazard of approximately 11.2-feet when they used an aluminum ladder that was not extended three feet above the second floor landing.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about April 4, 2018, at 2843 Shoals Hill Court, Dacula, Georgia:  Three employees performing framing work were exposed to fall hazards of approximately 22.4-feet and greater when they worked from the trusses and unprotected edges of a house without using fall protection.   Moises Degante dba ALK Framing Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1068191, citation number 1, item number 1, and was affirmed as a final order on September 23, 2015, with respect to a workplace located at 511 Starling View Circle, Grayson, Georgia.
