29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:   Job site, 641 Guenevere Drive, Pittsburgh, PA: On or about April 6, 2018, during roofing activities, the employer did not ensure that appropriate fall protection systems/devices were used where fall hazards of at least approximately 20 feet existed.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   Job site, 641 Guenevere Drive, Pittsburgh, PA: On or about April 6, 2018, during roofing activities, the employer did not ensure that the ladder, used to gain access to/from the roof was properly extended, exposing employees to fall hazards of up to approximately 20 feet.

29 CFR 1926.20(b)(2): The employer did not insure that programs existed that provided for the frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:   Job site, 641 Guenevere Drive, Pittsburgh, PA: On or about April 6, 2018, during roofing activities, the employer did not ensure that proper safety protocols were being followed at the job site. Deficiencies noted during the inspection included, but were not limited to, failing to use appropriate fall protection at all times while working on the roof, not ensuring that ladders were properly extended beyond the roofline and permitting employees who operated nail guns to do so without using eye/face protection.
