On or about April 10, 2018, and times prior thereto, employees performing servicing and maintenance, including, but not limited to cleaning and clearing jams from the maintenance chipper, did not follow the energy control procedures before having hands near the moving parts of the chipper.

On or about April 10, 2018, and times prior thereto, the rotating parts of the conveyor were not guarded to protect employees from the nip points created by the rollers of the conveyor. b) Maintenance Chipper area. On or about April 10, 2018, and times prior thereto, the chipper was not guarded to protect employees from the flying wood chips created by the chipper.

29 CFR 1910.212(a)(3)(ii): The point of operation guarding device was not so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: a) Maintenance Chipper area. On or about April 10, 2018, and times prior thereto, the point of operation was not guarded to prevent employees from being able to reach hands inside the chipper. On April 4, 2018, and employee suffered a fingertip amputation when his hand came in contact with the blades of the chipper.
