29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) were not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    a) International Timber & Veneer, maintenance chipper room.  On or about April 10, 2018, and times prior thereto, employees performing servicing and maintenance, including, but not limited to cleaning and clearing jams from the maintenance chipper, did not follow the energy control procedures before having hands near the moving parts of the chipper.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points, rotating parts and flying chips:  a) Maintenance Chipper area.  On or about April 10, 2018, and times prior thereto, the rotating parts of the conveyor were not guarded to protect employees from the nip points created by the rollers of the conveyor.  b) Maintenance Chipper area.  On or about April 10, 2018, and times prior thereto, the chipper was not guarded to protect employees from the flying wood chips created by the chipper.

29 CFR 1910.212(a)(3)(ii): The point of operation guarding device was not so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle:   a) Maintenance Chipper area.  On or about April 10, 2018, and times prior thereto, the point of operation was not guarded to prevent employees from being able to reach hands inside the chipper.  On April 4, 2018, and employee suffered a fingertip amputation when his hand came in contact with the blades of the chipper.

29 CFR 1904.41(a)(2): Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. The establishment had 20 or more employees but fewer than 250 employees at some time during the previous calendar year, and the establishment was classified in an industry listed in appendix A to subpart E of this part, but did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. The employer did not submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form:  a) International Timber & Veneer, located at 75 McQuiston Drive in Jackson Center, PA.  On or about April 10, 2018, and times prior thereto, the employer failed to electronically submit information from the 2016 OSHA Form 300A or equivalent by 12/15/2017. The NAICS for the establishment is 32121 (Hardwood Veneer and Plywood Manufacturing) and the company employed approximately 190 employees during calendar year 2016.
