OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burns and other related injuries from contact with hot steam pipes:  a.) International Timber & Veneer, LLC, located at 75 McQuiston Dr, Jackson Center, PA 16133: On or about 4/28/2023, the employer did not ensure boiler operator employees were protected from severe burn hazards due to potential contact with non-insulated or unguarded metal steam pipes. The metal steam pipe temperature measured up to 208.0 degrees Fahrenheit during boiler blowdown and ran horizontally approximately 6 feet above the walking/working surface alongside the wood feed end of the Hurst combination boiler.  b.) International Timber & Veneer, LLC, located at 75 McQuiston Dr, Jackson Center, PA 16133: On or about 4/28/2023, the employer did not ensure boiler operator employees were protected from severe burn hazards due to potential contact with non-insulated or unguarded metal steam pipes. The metal steam pipe temperature measured up to 281.0 degrees Fahrenheit and ran vertically on the side of the Hurst gas boiler within approximately 6 feet of the walking/working area.  c.) International Timber & Veneer, LLC, located at 75 McQuiston Dr, Jackson Center, PA 16133: On or about 4/28/2023, the employer did not ensure boiler operator employees were protected from severe burn hazards due to potential contact with non-insulated or unguarded metal steam pipes. The metal steam pipe temperature measured up to 190.0 degrees Fahrenheit and ran horizontally on the side of the Hurst gas boiler within approximately 6 feet of the walking/working area.  d.) International Timber & Veneer, LLC, located at 75 McQuiston Dr, Jackson Center, PA 16133: On or about 4/28/2023, the employer did not ensure boiler operator employees were protected from severe burn hazards due to potential contact with non-insulated or unguarded metal steam pipes. The metal steam pipe temperature measured up to 298.0 degrees Fahrenheit and ran vertically on the side of the control panel below the deaerator tank within approximately 6 feet of the walking/working area walking/working area.  ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

29 CFR  1910.147(f)(4):During shift or personnel changes, specific procedures were not utilized to ensure the continuity of lockout or tagout protection, including provision for the orderly transfer of lockout or tagout devices between off-going and oncoming employees, to minimize exposure to hazards from the unexpected energization or start-up of the machine or equipment, or the release of stored energy.   a.) International Timber & Veneer, LLC, on or about 4/28/23 and times prior thereto: employees periodically conducted service, maintenance, and repairs on equipment including but not limited to the Boiler.  The energy control procedures developed by the employer for use during Shift and Personal Change conditions was not specific and/or require specific procedural steps, to ensure the continuity of protection between employees.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). 29 CFR  1910.147(e)(3): When the authorized employee who applied the lockout or tagout device was not available to remove it, and the device removal is completed under the direction of the employer, specific procedures and training were not developed, documented and incorporated into the employer's energy control program:   a.) International Timber & Veneer, LLC, on or about 6/28/23 and times prior thereto: employees periodically conducted service, maintenance, and repairs on equipment including but not limited to the Boiler.  The energy control procedure developed for use during removal of a lockout device by someone other than the authorized employee who applied it situations, were not specific and/or require specific procedural steps prior to and after the removal of the lock.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

International Timber & Veneer, LLC, Boiler Room, on or about 4/28/23 and times prior thereto: Employees were working and walking around exposed rotating parts located approximately 3 feet above the floor.  The rotating parts include but are not limited to, a pully system used to power an auger under the feed line conveyor.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

International Timber & Veneer, LLC, Boiler Room, on or about 4/28/23 and times prior thereto: Employees were working and walking around exposed rotating parts located approximately 3 feet above the floor. The rotating parts include but are not limited to. chain and sprocket system used to power an auger under the feed line conveyor  B.)International Timber & Veneer, LLC, Boiler Room, on or about 6/28/23 and times prior thereto:  the guard for chain and sprocket drive was inadequate as there was a hole in the guard.  Chain and Sprocket located approximately 3 feet above the ground. 29 CFR  1910.151(c):Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a.) International Timber & Veneer, LLC, located at 75 McQuiston Dr, Jackson Center, PA 16133: On or about 4/28/2023, boiler operator employees work around corrosive chemicals while performing work tasks.  A paper/plastic bag was used to cover the entirety of the eyewash station. The bag could potentially obstruct use of the eyewash station during an emergency.  NO ABATEMENT REQUIRED
