29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use. In the cast lip area near the paint booth, the eyewash station was blocked potentially exposing employees to eye hazards while spraying paint containing a category 1 corrosive material.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure. On the west side of production area, a power cord attached to compressed air pipes with zip ties was used as permanent wiring to energize an electrical power cord roller exposing employees to electrical shock hazards.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). On the west side of the production area, employees used portable grinders and drills energized by a power cord with the outer sheathing split with visible internal wiring exposing employees to electrical shock hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. In the machine shop area, employees operated a band saw to cut metal material with an unguarded portion of the blade exposing them to amputation hazards. Hensley Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 1060821, citation number 01, item number 001 and was affirmed as a final order on October 15, 2015, with respect to a workplace located at 816 Jerkins St., Mansfield, Louisiana 71052.
