29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) On or about 04/10/18, on a jobsite located at the back of a house, 148 West Ave., Buffalo, NY: An employee was on a the pik of a Ladder Jack Scaffold erected on the back side of the house taking measurements at a window. The employee was exposed to a potential fall of approx. 20 ft. from the pik. No Fall protection system was being utilized. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 04/10/18, on a jobsite located at the back of a house, 148 West Ave., Buffalo, NY: Employees used an extension ladder to access the pik of a ladder jack scaffold. The siderails of that extension ladder only extended about 7 inches above the edge of the pik. NO ABATEMENT CERTIFICATION REQUIRED
