29 CFR 1910.22(a)(1): The employer failed to provide a clean and sanitary condition for all places of employment, passageways, storerooms, service rooms, and walking-working surfaces.    On April 11, 2018, the employer failed to ensure that the worksite was maintained in a clean and orderly condition. Boxes were observed all over the work floor creating a tripping hazard and making it difficult to walk/access the areas.      (a) 1st floor, North side of building was not maintained in a clean and orderly condition. Boxes were observed stacked on the ground throughout the area.    (b) 2nd floor, South East side of building was not maintained in a clean and orderly condition. Boxes were observed stacked on the ground throughout the area.    (c) 2nd floor, North East side of building was not maintained in a clean and orderly condition. Boxes were observed stacked on the ground throughout the area.    (d) 3rd floor, Amazon Room was not maintained in a clean and orderly condition. Boxes were observed stacked on the ground throughout the area.      In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.22(c): The employer did not provide, and ensure each employee used, a safe means of access and egress to and from walking-working surfaces:  (a) 1st floor, North side of building: On April 11, 2018, the employer failed to ensure that safe means of access and agrees was provided for employees. Boxes were observed all over the work floor/aisle and making it difficult to access these areas.    In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. (a) 1st floor, Blanket Room - On April 11, 2018, the exit door could not be opened from inside at all times because it had been lock on it. 29 CFR 1910.27(a): Scaffolds used in general industry must meet the requirements in 29 CFR part 1926. subpart L.    29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units.    (a) Warehouse - On April 11, 2018, the employer did not ensure that the painters scaffolds used to access boxes  were  fully planked and safe to use.      In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification.
