(a) Warehouse - On September 7, 2018, the employer did not ensure that the UST Model U57101 scaffold used to access boxes were fully planked and safe to use.      Margolin Shoes, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.451(b)(1) which was contained in OSHA Inspection Number 1308117, Citation number 1, Item number 3 and was affirmed as a final order on June 6, 2018 with respect to the workplace located at 2701 N. Kildare Ave. Chicago, Illinois 60639.       In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.
