29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:     a) On or about April 12, 2018, a fall protection anchorage device designed to support 5,000 pounds per one employee was being used to support two employees simultaneously during roofing work while the single anchor was only partially attached to the ridge of the roofline.  b) On or about April 12, 2018, an anchor used by a single employee as part of a fall protection system was not connected completely to ensure the device would be capable of supporting at least 5,000 pounds of force.
