29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  The employer does not maintain a program to provide for frequent and regular inspections of the job site, and does not designate any competent person to ensure that workers at the site are protected from recognized hazards, including but not limited to, fall hazards.    a)  This violation most recently occurred on September 10, 2018, at 4405 Maple Ct., Rolling Meadows, Illinois, where a competent person did not inspect the worksite to ensure that workers were protected from recognized hazards.  Abatement Note:  To abate this hazard in the future, the employer must initiate and maintain programs, including frequent and regular inspections of the worksite by a designated competent persons, to ensure that workers are protected from fall hazards.  Pursuant to 29 CFR 1903.19 the employer must submit documents describing the steps it is taking to ensure compliance, including the names and qualifications of the persons it is designating as competent and documentation that jobsite inspections are regularly occurring.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  The employer does not provide a training program which covers fall hazard recognition and the procedures to be followed in order to minimize those hazards for employees who are exposed to fall hazards while performing roofing work.  a)  This violation most recently occurred on September 10, 2018, at 4405 Maple Ct., Rolling Meadows, Illinois, where employees were exposed to fall hazards while performing roofing work and the employer did not provide a training program to recognize fall hazards and the procedures to be followed in order to minimize those hazards.  Abatement Note: To abate this violation, the employer must initiate and provide a training program for each employee exposed to fall hazards that would allow each employee to recognize the fall hazards and the procedures for minimizing the hazards. 29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b):      The employer does not protect each employee required to work from elevations during residential construction activities 6 feet or more above lower levels by providing and ensuring the use guardrail systems, safety net system, or personal fall arrest system, or providing alternative fall protection measures under provisions of paragraph 1926.501(b):   a)  This violation was most recently occurred on September 10, 2018, at 4405 Maple Ct., Rolling Meadows, Illinois, where employees performing roofing work were not protected from fall hazards of approximately 20 feet to the ground.   Abatement Note: To abate this violation, the employer ensure that its employees performing roofing work are protected from falls, and that no employee is allowed onto a roof for than 6 feet high without adequate fall protection.  Pursuant to 29 CFR 19103.19, the employer must submit documents describing the steps it is taking to ensure it is taking to ensure compliance, including a description of how these steps protect its employees from fall hazards.  Jonathan A. Arriola dba JAA Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1308180, citation 1, item 2b, and was affirmed final order on May 14, 2018, with respect to a workplace located at Ensile Drive Lot 97, Vernon Hills, IL 60061.
