Paris Road, Columbia MO 65202-2301: Employees were exposed to fall and struck-by hazards. The supported scaffold frames were not provided with base plates and mud sills or other adequate foundation. Scaffold frames were either supported by base plates or mud sills but not both, exposing employees to a scaffold collapse. The fall hazard was greater than 32 feet from the upper working level of the scaffold.
