29 CFR 1926.451(b)(4): Each end of a platform, which was not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches:   At the construction site located at 2302 E. Paris Road, Columbia MO 65202-2301:  Employees were exposed to a fall hazard.  The supported scaffold was not cleated or restrained by hooks.  Exposing the employees to a fall hazard due to possible displacement of the walking/working surface/plank of 32 feet 8 inches.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:  At the construction site located at 2302 E. Paris Road, Columbia MO 65202-2301:  Employees were exposed to fall and struck-by hazards.  The supported scaffold frames were not provided with base plates and mud sills or other adequate foundation.     Scaffold frames were either supported by base plates or mud sills but not both, exposing employees to a scaffold collapse.  The fall hazard was greater than 32 feet from the upper working level of the scaffold.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  At the construction site located at 2302 E. Paris Road, Columbia MO 65202-2301:  Employees were exposed to a fall hazard.  Employees engaged in masonry work, applying rock clad to an exterior wall of the building, were exposed to a fall hazard.  Employees were exposed to a fall hazard of up to 32 feet from the working platform of the scaffold to the ground below.  A complete guardrail system or other means of fall protection was not provided.

Paris Road, Columbia MO 65202-2301:  Employees were exposed to a fall hazard.  Employees engaged in masonry activities, stucco application to an exterior wall of the building, were exposed to fall hazard of 32 feet from the upper working platform to the ground below.  A complete guardrail system or other means of fall protection was not provided on the scaffold.
