29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  At the construction site located at 1412 Cinnamon Hills Rd in Columbia, Missouri:  Employees engaged in masonry activities using tools and equipment were exposed to struck-by hazards in that eye protection was not provided while installing masonry materials.

29 CFR 1926.451(c)(3): Supported scaffold poles, legs, post, frames, and/or uprights were not plumb and braced to prevent swaying and displacement:  At the construction site located at 1412 Cinnamon Hills Rd in Columbia, Missouri: Employees working from a supported scaffold were exposed to a fall hazard in that the scaffold was missing cross-bracing, exposing employees to a scaffold collapse and subsequent fall hazard of up to 25 feet.

29 CFR 1926.501(c): When employees were exposed to falling objects the employer did not require employees to wear hard hats and did not implement measures to prevent objects from falling from higher levels as set forth in (c)(1)-(3) of this section:  At the construction site located at 1412 Cinnamon Hills Rd in Columbia, Missouri:  Employees engaged in clean up activities below employees working at heights were exposed to struck-by hazards in that employees at the lower levels were not provided hard hats.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  At the construction site located at 1412 Cinnamon Hills Rd in Columbia, Missouri: Employee on the top level of the scaffold was exposed to fall hazards and was not protected from falling by the use of an adequate guardrail system.  Camouflage Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number1308210, citation number 1, item number 3 and was affirmed as a final order on 9/4/18, with respect to a workplace located at2302 E Paris Road in Columbia MO.
