29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a.  On or about April 12, 2018, at the above addressed jobsite, employees conducting roofing operations, twenty (20) feet above the ground, were not protected from falling.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not  independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee:  a.  On or about April 12, 2018, at the above addressed jobsite, employees conducting roofing operations from a steep sloped roof, twenty (20) feet  above the ground,  had lifelines secured to roof anchor plate with one side without nails and the other side of the plate having seven (7) nails, thereby exposing the employees to fall hazards.

The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.   a.  On or about April 12, 2018, at the above addressed jobsite, employees conducting roofing operations, approximately twenty (20) feet above the ground, were wearing harnesses with the attachment point at the employee's side "d" ring, thereby exposing the employees to the hazards associated with falls.
