29 CFR 1926.102(a)(1):The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about February 19, 2026, at 8000 Town Center Drive, Broadview Heights, 44147, the employer failed to ensure employees utilizing a pneumatic nail gun to secure weatherproofing to a residential was protected by eye/face protective devices in that employees were wearing no face protection.

29 CFR 1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about February 19, 2026, at 8000 Town Center Drive, Broadview Heights, OH 44147, the employer failed to ensure employees installing weatherproofing on a commercial roof were protected from a fall of approximately 12 feet from a 4:12 pitch roof in that no personal fall arrest systems/devices were utilized.
