29 CFR 1910.147(c)(4)(i): Procedures that were developed, and documented, were not utilized for the control of potentially hazardous energy when employee was engaged in activities covered by this section. (a) W.A. Whitney Plasma Punch Press: On or about April 25, 2018, employees did not utilize established procedures to isolate energy sources to the Plasma Punch machine, prior to clearing a stuck part from the feed chute to the slag conveyor. This exposed employee(s) to pinch point/crushing hazards, and amputation injuries.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about April 12, 2018, employees who worked at or around the American (Model #812 equipment #MT 409) Engine Lathe were not protected from the rotating 48 inch face plate and attached work piece. This exposed employees to struck by and caught on hazards. (b) Employees who operate the Whitney plasma punch (Model #661 serial #665-200-41955), located in Fabrication & Welding Bay #3, are exposed ingoing nip points, struck by hazards, and sparks, while standing on the burn table and work piece during machine operation. (c) Employees who operate the Chicago brake press (Model # 10000VC18 serial #H1141) located in Fabrication & Welding Bay #1, are exposed to struck by hazards when forming specialty metals that break or shatter. No barrier guarding is utilized to contain fragments of metals that are projected from the press. (d) Employees who operate the Accurpress 1000 ton brake press (Equipment # MT 707) located in Fabrication & Welding Bay #3 area, are exposed to struck by hazards when forming specialty metals that break or shatter. No barrier guarding is utilized to contain fragments of metals that are projected from the press.

29 CFR 1910.252(b)(2)(iii): Workers or other persons adjacent to the welding areas were not protected from the rays by noncombustible or flameproof screens or shields or required to wear appropriate goggles. (a) Fabrication & Welding Bay #3: On or about April 12, 2018, employees were exposed to welding rays while working in and around the welding bay operations located in the bay #3 area. Several welding operations did not have shielding in place to protect near by employees performing non-welding work duties.
