29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) at the workplace - there was not a lockout tagout program with machine/equipment-specific energy control procedures where employees performed maintenance and repairs on machinery including, but not limited to, S&S Model ZLD, 50" Flexo Folder Gluer serial #20681 and other flexi-gluers, exposing employees to hazards including, but not limited to, mechanical, electrical, and amputation hazards; on or about March 29, 2018.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are barrier guards, two-hand tripping devices, electronic safety devices, etc: (a) S&S Model ZLD, 50" Flexo Folder Gluer, Serial #20681 - employees performing changes and adjustments during machine setup such as, but not limited to, changing knives, were exposed to a crushing hazard when standing between the rollaway sections of the machine; observed on or about March 29, 2018.
