29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks: a) On or about August 26, 2025, at the production area of the jobsite located at 1101 E 33rd Street, Hialeah, FL, 33013, an employee was exposed to an amputation hazard while working in close proximity of a powered roller conveyor ( Servicing: Machine 148 - Marquip ward united - Serial No.42223 and Mosca SQ4 strapping machine - Serial No. 506431) missing a section of the conveyor motor side guard panel. b) On or about August 26, 2025, at the production area of the jobsite located at 1101 E 33rd Street, Hialeah, FL, 33013, employees were exposed to entanglement hazard while working in close proximity of a powered roller conveyor ( Servicing: Machine 148 - Marquip ward united - Serial No.42223 and Mosca SQ4 strapping machine - Serial No. 506431) lacking the rail guard for the rotating shaft. SupplyOne Tampa Bay, Inc, Corp was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1505359, citation number 1, item number 1, and was affirmed as a final order on 05/19/2021, with respect to a workplace located at 3939 Airway Circle, Clearwater, FL, 33762.
