29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about April 11, 2018, at the worksite located at 5510 Pershing Avenue, Saint Louis, Missouri, an employee for Armor Siding LLC was observed, installing siding from a JLG Lift without wearing a body belt with a lanyard attached to the boom or basket. The fall distance from the lift to the ground was approximately 40 feet from the ground below.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet or more above lower levels and the inside bottom edge of the wall opening was less than 39 inches (1.0 M) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system. On or about April 11, 2018, at the worksite located at 5510 Pershing Avenue, Saint Louis, Missouri, an employee for Armor Siding LLC was observed painting a closet on a third-floor balcony of a multi-family residential structure without guardrails or any other form of fall protection. The fall distance was approximately 30 feet above the ground below.
