29 CFR 1910.137(c)(2)(viii): Electrical protective equipment was not subjected to periodic electrical tests. Test voltages and/or the maximum intervals between tests were not conducted in accordance with Table I-4 and Table I-5:     On or about April 16, 2018 through May 23, 2018, rubber insulating gloves provided for use to a refrigeration maintenance employee had not been electrically tested (or replaced) at six month intervals to ensure that the dielectric properties of the gloves were adequate to protect from contact with energized parts while performing energized tasks, including voltage testing.  The gloves were last tested as required in June, 2013.     Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:     Production Area, 3rd Shift Sanitation - On or about April 16, 2018 through May 23, 2018, employees who performed cleaning and sanitation of the Variegate Vats were exposed to injury from the unexpected start-up of the agitator blades.   The employer did not adequately train employees to ensure that they acquired the knowledge and skills required for the application and use of energy controls including energy isolation, lockout and verification prior to inserting their hands and arms into vats for safe cleaning and removal of agitator blades.  Employees were trained to merely stop the blades.       Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):        Production Area, 3rd Shift Sanitation - On or about April 16, 2018 through May 23, 2018, employees who performed cleaning and sanitation of the Variegate Vats were exposed to struck-by hazards from the unexpected startup of the agitator blades that were likely to cause laceration/amputation.  Employees inserted their hands and arms into vats for cleaning and removal of agitator blades and were not required to control hazardous energy sources in the following sequence:     1) turn off and shut down the machine,   2) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s),     3) affix lockout or tagout devices to each energy isolating device,   4) verify that isolation and deenergization of the machines or equipment have been accomplished.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.305(g)(1)(ii): Flexible cords used as permitted in paragraphs (g)(1)(ii)(C), (g)(1)(ii)(G), or (g)(1)(ii)(I) of 29 CFR 1910.305 were not equipped with an attachment plug and energized from an approved receptacle outlet:     Production Area, Packaging Line 1 - On or about April 16, 2018 through May 23, 2018, the Variegate Vat was permanently wired with a flexible cord that was not equipped with an attachment plug and was not energized from a receptacle outlet, creating a fire hazard.     Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
