29 CFR 1926.51(f)(1): The employer did not provide adequate washing facilities for employees engaged in the application of paints, coating, herbicides, or insecticides, or in other operations where contaminants may be harmful to the employees. Such facilities shall be in near proximity to the worksite and shall be so equipped as to enable employees to remove such substances:    South side: There was no soap and no hand towels or other drying methods available for employees that used Portland cement products in the process of laying concrete blocks.

29 CFR 1926.1053(a)(2): Ladder rungs, cleats, and/or steps were not parallel, level, and uniformly spaced when the ladder was in position for use:    West side: A job-made wooden extension ladder, approximately 7-feet in length, with non-uniformly spaced rungs was used to access the next working level platform.  Employees were exposed to fall hazards.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement:  Grease Trap Manhole: Rebar with damaged mushroom style plastic caps protruded approximately 4-inches from the top surface of the manhole.  Employees were exposed to impalement hazards.  Isagani B. Baluyut Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.701(b), which was contained in OSHA inspection number 1271776, citation number 1, item number 1 and was affirmed as a final order on December 12, 2017, with respect to a workplace located at Harbor View Building - Lot 791, Corner of Soledad Ave. & Father Duenas Ave., Hagatna, GU 96910.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  Temporary power area: The outer insulation of an extension cord that was used to power a circular saw was separated from the plug, exposing the employee to electrical and fire hazards.
