29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    North side: A job made wooden extension ladder used to provide access to the footing did not extend at least three feet above the upper landing surface.  Employees were exposed to fall hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  Temporary power area: The outer insulation of the Milwaukee Abrasive Cut-Off Machine power cord was separated from the plug.  Employees were exposed to electrical and fire hazards.  Isagani B. Baluyut Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.405(g)(2)(iv), which was contained in OSHA inspection number 1197000, citation number 1, item number 1 and was affirmed as a final order on January 25, 2017, with respect to a workplace located at Lot 2140-4-R2NEW-1NEW-3, Tamuning, GU 96913.

Employees were exposed to strain and fall hazards.
