29 CFR 1926.1412(d)(1): Visual inspections were not performed by a competent person prior to each shift for apparent deficiencies of the equipment that was used: South side: The Grove Telescopic Boom Crane, Model TMS-2500 was not visually inspected for defects before placing the crane in service. The parking brake was defective, thus exposing employees to struck-by and/or caught-between hazards.

Grove Telescopic Boom Crane, Model TMS-2500: A load chart was not posted or readily available in the crane cab. Employees were exposed to crush and struck-by hazards.
