29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service    On or about April 17, 2018, employees were exposed to struck-by hazards when employees were not mandated to inspect short fork pallet jacks, long fork pallet jacks and standup forklifts on each shift that the powered industrial trucks were driven.      The was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.178(q)(7), which was contained in OSHA inspection number 1244764, citation number 1, item number 5, and was affirmed as a final order on January 3, 2018, with respect to a workplace located at 2940A Pacific Drive, Norcross, GA.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces were inspected, regularly and as necessary, and maintained in a safe condition:  On or about April 17, 2018, employees were exposed to struck-by and tripping hazards due to the following:  a. floor holes that had not been maintained in a safe condition created tripping hazards for employees walking while engaged in warehousing activities.  b. floor holes that had not been maintained in a safe condition exposed employees engaged in warehouse activities to struck-by hazards in that powered industrial trucks drive over them while carrying pallets of light bulbs.  The employer was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.22(d)(1), which was contained in OSHA inspection number 1244764, citation number 1, item number 2, and was affirmed as a final order on January 3, 2018, with respect to a workplace located at 2940 Pacific Dr., Norcross, GA  30072.
