Addison Street in Chicago, IL was maintained in a clean and dry condition. The aisle floor near the intermediate bulk containers had a puddle of liquid with a pH of 10.

Addison Street in Chicago, IL was available for immediate use. The eyewash station/safety shower was blocked by stored intermediate bulk containers of chemicals.
