29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a) On or about April 16, 2018: An employee was observed operating a pneumatic nail gun to install fascia's without using any type of eye or face protection. The employee was exposed to eye and face injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about April 16, 2018: On a 6:12 pitch roof an employee was observed installing fascia's and was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was employee provided with an alternative fall protection measure, the employee was exposed to a 34-feet fall hazard.  b) On or about April 17, 2018: An employee was observed installing bracing elements to roof trusses was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was employee provided with an alternative fall protection measure, the employee was exposed to a 34-feet fall hazard.  c) On or about April 17, 2018: On a 6:12 pitch roof an employee was observed installing roof sheathing and was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was employee provided with an alternative fall protection measure, the employee was exposed to a 34-feet fall hazard.  d) On or about April 17, 2018: On a 6:12 pitch roof an employee was observed cutting wood and was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was employee provided with an alternative fall protection measure, the employee was exposed to a 34-feet fall hazard.  e) On or about April 17, 2018: An employees was observed installing bracing elements and was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was employee provided with an alternative fall protection measure, the employee was exposed to a 12-feet fall hazard.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) On or about April 17, 2018: Three employees were observed performing roofing activities at heights higher than 6-feet from next lower level which exposed them to a fall hazard and such employees were not provided with training that will allow them to recognize such hazards and equipped them on how to minimize them.
